Restaurant / food-facility permit

Food Facility Health Permit (Restaurant)
Annual by seats: under 26 $998; 26-50 $1,129; 51-75 $1,194; over 75 $1,325. Take-out $998-$1,031. Plus a one-time $207 new-facility application fee.

Plan review — required

Food truck / mobile permit

Mobile Food Facility (MFF) Permit
Annual by type: catering truck/trailer $1,058; enclosed truck low-risk $657; espresso $657; prepackaged ice cream $444; delivery-only $555; cart ~$500. Plus one-time $207 application fee.

Commissary mandatory (Part D agreement); ACDEH keeps an approved-commissary list. No commissary = no MFF permit.

Watch out for

  • EH food-facility plan approval must be obtained BEFORE the local Building Dept will issue building permits.
  • Commissary is mandatory for all mobile units; new builds need a full plan-check package (site plan, equipment cut sheets, plumbing schematic, hot-water worksheet, menu).
