Risk level is assigned at plan review. An additional catering permit is +$200. Establishments must have a certified Food Safety Manager on staff (Utah Code Title 26, Ch. 15a).

New/remodeled mobile units submit plans, equipment list, menu and commissary letter, and pass a pre-operation inspection before licensing.

Temporary events: $70 event permit; $225 multi-event permit (valid one year); $75 day-of-event late processing fee.
Food handler permit is $20 per person, valid three years.
